Included in this page are outlines and sample forms to assist county election commissions in evaluating polling sites as required by the "Americans with Disabilities Act" (ADA). The forms include a "Self-Evaluation Plan" (SEVP 01), "ADA Transition Plan Outline" (TPO 01), "Polling Site Accessibility Checklist" (PSACL 01), "Polling Site Accessibility Summary" (PSAS 01), and an "ADA Discrimination Complaint Form" (COMPL 01). Also included is a sample "ADA Transition Plan Outline," a sample "Grievance Procedure," and a sample "Publication Notice".

  All of these forms presented here are in ".pdf" , short for Printer Description File, format for maximum portability and convenience. If you do not already have a copy of Adobe's Acrobat Reader, you may download a free copy by following this link. You will need Adobe Acrobat to print these sample files.

  Upon completion of the "Self-Evaluation Plan" (SEVP 01), "ADA Transition Plan Outline" (TPO 01), "Polling Site Accessibility Checklist" (PSACL 01), and the "Polling Site Accessibility Summary" (PSAS 01), return copies of all of the above forms EXCEPT the "Polling Site Accessibility Checklist" (PSACL 01), to the State Board of Election Commissioners (SBEC) by November 30, 1997. Follow-up dates are listed on page 4 of this manual.
