All renewal information was sent out on October 30, 2009. There are approximately 6,500 facilities/individuals renewing at this time.
If you do not wish to renew online, you must submit a written request for a paper renewal form to be mailed to you. You may fax, email or mail the request to the ASBP office. Your request MUST include:
- Name
- License Number (Your license number is located on the front of this card.)
- Mailing Address (if you would like the form mailed) OR Email Address (if you would like the form e-mailed).
We will not fax paper renewal forms.
Please type, or print legibly, all requests. Any request that is unclear or cannot be read will not be processed. Please allow 2 weeks for the paper renewal form to be mailed to you.
